Initial Assessment

A member of our Management Team will visit your home at a convenient time to assess your needs. We encourage you to have a family member or friend with you during this visit.

What We Do: Discuss and identify the support you require at home. Conduct risk assessments to ensure safety for you and our Care Team. Perform a moving and handling risk assessment, if needed, to identify equipment or techniques for your care.

What We Offer: A thorough, personalised assessment that considers your safety and wellbeing.

Tailored Care Planning

Using the information gathered during the assessment and input from other agencies, we will create a care package tailored to your unique needs and preferences.

What to Expect: A personal care plan setting out how services will be delivered to support your lifestyle and goals. A flexible plan that adapts to your changing needs. Your care plan will be available in a language and format you understand.

Continuous Monitoring & Reviews

Our Care Team will monitor your care plan daily to ensure it continues to meet your needs. If any changes are identified, they will notify the Management Team immediately.

Key Features: A review of your care plan within the first six weeks and every three months thereafter by a Senior Manager. Ongoing adjustments to your care plan as required. Collaborative efforts to minimise any risks identified.
